Here's the place to find information regarding your account, billing, dental insurance, forms, and appointments.
There are many dental insurance companies and just as many plans within the companies. While our office accepts most dental insurances, it is the responsibility of the patient to check to make sure that dental work performed in our office will be covered.
To avoid any misunderstandings regarding your dental insurance coverage, we ask that you read your policy carefully and make yourself familiar with not only the benefits, but also any limitations.Most insurance companies only pay a portion of the dental fees up to a certain annual maximum. There are countless agreements between insurance companies and patients, and each contract provides different benefits.
Our office will bill your insurance company for you, and is happy to assist you with any follow-up or clarification that is required by the insurance company.
While we advocate for you with the insurance company, the fees are the patient's responsibility.

The patient is ultimately responsible for their account. We request that unless previous arrangements have been made, that payment be made at the time of service.
Many of our patients have a portion of their dental care paid by an insurance carrier.. As a courtesy to you, we will be happy to submit your claim forms for you. The insured patient's co-payment is requested at the time of treatment. charge of 1.5% per month will be added to unpaid balances after 30 days regardless of insurance disposition.
For the convenience of our patients, we do accept Master Card, or VISA.

At your first appointment, the doctor will do a thorough examination of your mouth for oral cancer, condition of your teeth, gums, and surrounding tissue. This information provides you with an accurate picture of health or any diseased state that may exist, as well as providing a baseline for any future developments in your oral health. At the completion of the exam, the doctor invites your discussion of your needs, and treatment options that best fit your situation. Is all this necessary?
Our practice stresses the importance of prevention of all forms of dental disease. Gum disease is the number one cause of tooth loss in the United States.
At your first appointment, the doctor will do a thorough examination of your teeth, gums and surrounding tissue. Dr. Collins may recommend further diagnostic tests as necessary. This information provides you with an accurate picture of any diseased state, as well as providing a baseline for any future developments in oral health.
Your hygiene appointments are one of the keys to healthy teeth, gums, and body. During this appointment, our hygienist will re-evaluate the health of your gums, clean and polish your teeth, and discuss the proper methods of flossing and brushing. Upon completion of this visit, you may schedule your next cleaning appointment.
Our office believes in maximizing the time used to examine you and answer your questions regarding your oral health and treatment options.As such, before your first visit, we ask that you fill out the forms included in the new patient packet (patient information form, medical and dental history, Privacy Practice notification and consent form (HIPPA).These forms are available for download, or the forms can be mailed to you for completion prior to the appointment.
Please bring the completed forms with you to your appointment along with your current dental insurance card.
Appointments
We work with our patients and their schedules to make the best appointment time available for the patient and the doctor's and hygienists' schedule. Appointments can be scheduled with Sylvia over phone or via email.
Missed appointments
Since your time with Dr. Collins and the hygiene staff is specifically set aside for you, two business days notice is required for rescheduling an appointment.
A missed appointment that is not given proper notice will be subject to a missed appointment fee.
We ask that our existing patients update their medical history once every five years. The form can be downloaded here, or it can be completed at the appointment.
